Make a real difference in your community
Are you passionate about helping others live safer, healthier, more independent lives? Join our team at Waythrough and support people facing challenges around mental health, substance use, housing or domestic abuse. This is more than just a job – it’s a chance to build meaningful relationships and create lasting change.
About the role
At Waythrough, we’re looking for a compassionate and motivated Social Isolation Support Coordinator to join our Community Services team. This is an exciting opportunity to join a new role supporting adults aged 55 and over who may be experiencing loneliness, social isolation, poor mental wellbeing or difficulties accessing their local community.
You’ll work directly with individuals on a one-to-one basis, helping them build confidence, improve their wellbeing and develop meaningful social connections. You’ll support people to identify their goals, explore opportunities within their communities and access the services and activities that can make a real difference to their lives.
As this is a new role, you’ll have the opportunity to shape how the service develops, build relationships with local organisations and demonstrate the positive impact this support can have. Your work will help us show commissioners the value of the service and its potential to improve outcomes for local people.
What will you be doing?
As a Social Isolation Support Coordinator, you will:
* Work with individuals to understand their needs, strengths, goals and the barriers they face.
* Develop personalised support plans focused on improving wellbeing, confidence and independence.
* Provide practical and emotional support to people experiencing loneliness, anxiety, low confidence or social isolation.
* Help individuals access local community groups, activities, volunteering opportunities and other support services.
* Accompany people to initial appointments or community activities where appropriate, helping them feel confident taking the next steps.
* Provide information, advice and signposting to relevant health, social care, housing, welfare and community services.
* Build positive working relationships with local organisations and community partners.
* Monitor progress, review support plans and maintain accurate records of outcomes.
* Recognise safeguarding concerns and follow the appropriate procedures to help keep people safe.
You’ll work independently, managing your own caseload while building a strong network of local connections to help people access the support they need.
What are we looking for?
We’re looking for someone who is approachable, empathetic and confident in building relationships with people from a range of backgrounds. You’ll understand the impact loneliness and social isolation can have on someone’s life and be passionate about helping people feel valued, connected and empowered.
You will have:
* Experience working with vulnerable adults, older people or individuals with wellbeing needs.
* An understanding of the causes and impact of loneliness and social isolation.
* Experience assessing individual needs and developing support plans.
* Knowledge of safeguarding principles and procedures.
* Excellent communication and interpersonal skills.
* The ability to motivate and empower individuals to achieve positive outcomes.
* The ability to work independently and manage your own caseload.
* A full driving licence and access to a vehicle, as required for the role.
A relevant qualification in health and social care, community development, counselling, advice and guidance, or a related field would be desirable.
Experience in social prescribing, community navigation or wellbeing support would also be beneficial. However, we welcome applications from people with transferable skills who can demonstrate the ability to engage effectively with individuals and build positive, trusting relationships.
